The usage is probably the result of kids raised on store-brand vanilla ice cream in the 70's. It was self-evident to me as a child that "vanilla" was the base (non-)flavor of ice cream, what you got if you didn't add any flavor to it. I think I didn't find out otherwise until high school or possibly even into college.

The weird thing is that I was familiar with vanilla extract. I just never put the two together because they were so obviously unrelated.
